Just last month I finished putting together
FindForms.com ( http://www.findforms.com ) which is a
legal forms search engine (which includes agreements
and contracts). We have spidered and indexed over
3,500 individual free legal forms from all over the
Internet. I belive that when I was spidering forms, I
came across some forms similar to what you are looking
for. You might want to give it a try. You can also find
