We are using a good one, called Central Ad Pro. We are
serving nearly a half a million impressions a day
through it and it works well and gives us all the
flexibility you need. It costs around $600. You can
get it from http://centralad.com/
The downside:
1) their customer support (after you have purchased)
stinks big time, meaning they just don't respond
2) when you run several ads on the same page, you have
to use (or generate) a different random number for
each ad space, otherwise the soft gets confused
and starts misdirecting clickthroughs.
