Ken ... I have personally done a lot of work looking
into this for our company. There is a lot of garbage
out there when it comes to IP targeting. Even good
solutions have problems (e.g., AOL users from across
the country who come through proxy servers in N.
Virginia).
Check out Ingeodesy (at http://www.ingeodesy.com).
They appear to be one of the more thoughtful players in
this space.
