measuring the effectiveness of newsletters

Hi,

I have to do some research about the effectiveness of
newsletters. Let's say I have 5.000 subscribers for my
newsletter. First of all I'd like to know how many of
them actually opened the newsletter. The problem is
that a lot of people read offline. Is there a way to
track those who are reading offline?

Secondly, I want to know what is the best place to put
a link, banner, button, ... in the letter: on top, in
the middle, at the end? Anyone experience with this?
