Dear Jim,

I saw this on the online ads list and I am interested
in how you preface this request for information so that
(1) you get the information and (2) you do not offend
your former subscribers. Are you sending the survey
with a letter saying we have unsubscribed you but
please tell us what we can do etc, etc or is it a
seperate letter?
