Fellow advertising professionals, I need your advice...

We would like to offer an Affiliate Program to drive
traffic to our site. We are considering both a CPC and
CPA mixed model. In our case, the CPC would pay for
referral traffic to the site. The CPA would pay for
membership sign-ups once on the site.

- Who are the Top 5 services offering management of
  these programs?
- What are the Top 5 software programs most recommended
  for managing a program like this internally?
