Good examples of rate sheets

Hello, everyone. I've been asked by NAPM-SV (National
Association of Purchasing Managers, Silicon Valley) to
put together a "really good" (their phrase) web page
talking about their rate structure and requirements for
banner advertising on the www.napmsv.com web site.
They're doing a simple random banner display, priced
using a monthly or quarterly fee for having your banner
added to the stewpot. We're still getting the kinks out
in the script, so right now banners are only appearing
on a couple of the pages at the moment (such as
http://www.napmsv.com/cf/listalpha_sbd_free.cfm ) but
they're the typical full banner size and will appear at
the bottom of each page.

I've browsed around a bit, and found quite a few good
rate sheets/explanation pages for sites pricing via
CPMs or click-through/CPA costs, but haven't found many
good examples of rate sheets for sites like this one.
This isn't a high-traffic site, but NAPM members are an
extremely targeted group of people (the purchasing
managers at companies like Sun Microsystems, Palm, and
other Silicon Valley companies). I would love to have a
few examples of "how other sites do it" to point my
NAPM contact person to. Anyone got any good URLs they'd
care to share? Any sites doing this type of advertising
want to brag about how nicely they've done their rate
page or explained how to upload the banner gifs?

And does anyone who'd be the type to advertise on a
site like this have any requests for the type of info
they'd like/need to see on this type of web page? Since
in this case there aren't any long tables showing the
various CPMs for all the different configurations of
banner buys, I keep looking at the small amount I've
written so far and thinking "there must be something
missing!"

Also, does anyone who's submitted banner ads to a web
site have preferences/comments about having an online
form with a "browse" button so folks can sign up to
advertise and send their GIF at the same time? A form
might be a handy way to deal with submission of the
GIFs, but these things always seem to have hidden
drawbacks.
