I've done a lot of advertising online (successfully).
Of course there are a lot of things that dramatically
affect CTR:

1. placement (top is not always the best - you want the
MOST read section)

2. Camouflage - does it LOOK like and ad? If it you can
make it look like content on the page that works well
(P.s. I HATE those bogus error message banners - the
CTR is good - the conversion afterward is really poor).

3. Copy/Offer - as varied as the universe but most
folks do a competent job

4. Incentives - we use the offer to get a free 35mm
MEIKAI camera as a premium for coming to our site and
signing up for a newsletter or two - this works wonders
for our CTR in all formats.
