Biggest issue surrounds the serving of the code to pop
the window up and the actual serving of the ad into the
window it self.

If you serve the code for the ad but the window doesn't
popup (ie: many adblockers void out the window.open()
function in JavaScript) you haven't served the ad but
your adserver will make it look as if you have.

You ideally need a secondary counting mechanism that
appears in the popped-up window itself, this would
then be identified when the ad/image is served into
the popped-up window and thus provide a far more
accurate count of actual ad deliveries.
