Tracking newsletter open rates

Steve Tyler asked how you do tracking in newsletter:

       You will need to be able to issue cookies, or
have dynamically embedded images (ie, 1x1 pixels) that
tell you if you if the person opens the newsletter.
There are tons of problems here, as many email programs
don't allow cookies, some people have them turned off,
some people read only in text and so on. According to a
recent survey, about 20% of the people that
were shown not to read my newsletter read it every week!

       Realistic open rates are impossible to know unfortunately.
