We use www.exacttarget.com and have recommended this to many clients.
You can see our newsletter put together through the ExactTarget system
in the link in my signature line.

It definitely has all the tools you asked for... also has the ability to
do profiling, random sampling and more. Just recently they added a
survey tool which allows you to track surveys through it as well. I have
also looked at ConstantContact and thought it seemed pretty good...
(they were the final two I looked at after weeding out others for either
quality or price issues) but haven't used it as I chose ExactTarget at
the time.
