Whether you call it a free service or not, and ad is still an ad. You are
promoting your website. (And looking at your site, the dentists don't have a
free "website," they have a page on your web site, which seems to be hoping
to make money on advertising sales, or perhaps sales of products or services
to dentists.)
If you want to reach that audience without spamming, the way to do it would
be to buy ads in newsletters that do reach dentists now, buy pay per click
and pay for placement search engine ads, and send old-fashion direct mail -
you have your own database to grab the addresses from.
