As an Account Exec., I can give you some info from the agency side...
Generally the person you're looking for is the Media Buyer or Account
Planner. These days, titles don't always reflect actual job
responsibilities (I usually make the media buys for clients I'm responsible
for) so I suggest that you ask for the person responsible for media buying.
Specify whether this is for online buys, magazine, radio, etc. (I'm
assuming online here) Sometimes different people are responsible for
different areas.

What I want to know for a media buy...
Your target audience
# Unique visitors to your site
# uniques to the page the ad will appear on
# of other ads that appear on the same page
How long will our ad appear for? How often will it cycle through?
Content focus for the page the ad will appear on
Ad mechanicals (size? dynamic or static? Link w/ source code?)
Billing format (CPC? CPM?)
Rate (naturally)
Ideally, we prefer our ad appear as the only ad on a page and display
continuously.
