I wouldn't do business with a company that I cannot access by phone
-- at the vert least I want to know where they are located so that I
feel I can get a phone number if I need to. Although I will probably
conduct all my business on-line with the company, I want the security
of knowing I can talk directly to a human if there is a problem.

Cliff's recommendation of an on-line form is good -- but please note
that there are some users who do not use the same browser and email
companies. I use IE as a browser, but Eudora for email. There are
many forms that deny me access to email communication because I'm
using IE but not Outlook.
