What should e-mail marketers do?

I almost never post but this is something I've been reading about in-depth
and have some first hand knowledge of. I think that A. yes there are
obviously TONS of lists out there with questionable sources (if I hear one
more time how they're going to enlarge another body part...). But also, B.
very often people with either forget that they opted into something or C.
not trust the opt-out option provided when they no longer wish to receive
mail; so many spammers out there have used the opt-out button to validate
addresses that now people are too hesitant to even try. So I guess this
leads me to the question, what are marketers supposed to do now? You use
only sources that are opt-in, you give them a chance to opt-out, and still a
subscriber can as easily choose to hit the "this is spam" button over any
others and that's it. How are others addressing this?
