The answer depends to some extent on what it is that you want opinions
about.
If you want to hear from people who use your site on a regular basis, and if
you have a newsletter, put a description of the survey and link to it in
that (along with a reason it's useful for the reader to spend time filling
out the survey).

If you want to survey first time visitors to a site, and you want to get
their reactions on general interests, put it somewhere near the tops of the
entry pages that get the most new visitors.

If you want to find out their reactions after they leave the site (or why
they haven't ordered, etc.) a popup exit survey (or link to one if it's
long) would be the way to do it.
