Hello Sarah,

I appreciate the initiative you are taking but I would warn you to carefully
make distinctions between surveys and initiatives to collect targeted email
addresses for later promotional efforts. I am a survey methodologist and
what we are observing is a decreasing response rate to surveys handled
through internet because of people willing to participate to surveys finding
themselves part of target email campains aimed to promote product and/or
services....

That is: surveying and email collection for promotional purposes should
always be distinct activities.
