Best Ad Formats & Types

I would like to hear your opinion, based on your experience, which
ad formats and types you find most cost-effective? Do you prefer
"ordinary" banner ads, Flash ads or text ads for your marketing
campaigns?

I'm interested both for web site advertising and email newsletter
advertising. I will appreciate if you also mention your average CTR
for your best ad formats & types.
