Huge site--no luck advertising

We're up to 190K unique visitors/month, 15+ million page views/month. We've
become one of the top three gay and lesbian sites online.

And we still cannot get a decent company to buy or sell our ad space. We
keep getting these $.40 ecpm companies approaching us.

Is this our fate? Is there any hope of decent advertising money?

I appreciate any advice, insight, help, and especially ad revenue! ;-)
