I consult for companies doing web surveys and I can surely advise a few
inexpensive software for surveying....

For what it concerns survey mode, you should be aware of more extreme
(very positive or very negative) comments gathered using a non-anonymous
questionnaire, while more realistic judgements and feedback can be
obtained using an anonymous questionnaire - e.g. administered by a
survey company on a randomly chosen sample of your customers.
