Also, what about various points between the top and bottom; for example,
banner ads near sub-headings of a page -- you know, what you jump to when
you click on a link at the top that's connected to an anchor within the
same page? Or even ad ad right within the text of an article, that's
directly related to the content? (You might want to use another form than
the standard banner ad for this, however.)

I suspect that what's effective and what isn't depends a lot on what the
page in question is offering and therefore, the usage patterns of that
particular page.
