ONLINE-ADS>> Types of Web ads?

Most often when we talk about Web ads we seem to be referring to
banners or similar small display-type ads. But in a recent issue of
Internet Marketing and Technology Report I was interested to find
this listing of formats:

Banner ad
Promotional content ad
Entitlement ad
Mini-site
Bridge page

I don't know if I've ever seen a widely-acknowledged classification of
ad formats.

I'm wondering:

1) If anyone has any formats to add to this list?

2) Other than banners, what do you think is meant by the other formats
on the list -- especially "entitlement ad" and "bridge page"? The
article just gave this brief listing, with no definitions.
