I know people in Japan, UK and other places who use a .com domain name
(sometimes because they connect through a head office, sometimes because
getting US domain names is cheaper and easier).
Probably this does not matter - you are maybe only selling to your own country.

But even the US has four time zones. If it is 7pm on one side of the
country, and 11pm on the other, how do you decide which ad to show?

While I agree that targetting by time would be useful, it does not seem
practical.
