Although this is a few months old, one of the most effective order-form
based advertising campaigns I've seen was run by Ozemail (one of the
biggest service providers in Australia). They placed themselves as 1 of
only 2 Australian online distribution points for Internet Explorer 4 and
when you clicked on the "Download Explorer" button you first taken to a
Pizza Hut ad which suggested that because it would take ages to download,
you should disconnect now, order a pizza, reconnect and by the time you'd
finished eating it your copy of Explorer would have downloaded. The
beautiful thing about this ad was that they had a captive audience. Nothing
else competing for space on the page, just their logo, a small amount of
copy and a "click to continue" button. Ok, so people are only likely to
visit that page once but the brand awareness from this campaign must have
been enourmous.
