The IAB has tried to approach the issue of "counting" in a method
that allows to comparable numbers between sites.

In particular when we are dealing with the concept of ad delivery, we try to
count at the server side (time of insertion) instead of client side (by gif
counting).
This largely avoids the question of cached vs. un-cached impressions.

We realize this isn't the best method since we would like to be able
to determine exactly what was seen by each user, but the currently available
technology doesn't allow for this in a comparable fashion.
