Suggestion: don't ask their name or e-mail addrss on the form and/or
make sure they know that the information they provide will be used for
demographics, not for knowing detailed information about them.

Maybe there can be two forms, one after the other: one will have
name/e-mail address, followed by the Thank You page and "Please take one
more moment to help us learn about our customers" blah, blah form. That
gives people the understanding of a "separate" yet "necessary" process
-- as long as your plea is good enough.
