I think Hotwired, http://www.hotwired.com, has an excellent registration
form. When users first go to register they see a simple registration form
with a few questions. After they submit that form they are registered, and
then they are redirected to a form with optional demographic questions.
Registration turns off many users, and keeping the first screen simple
allows Hotwired to draw in as many users as possible.

When you create your registration forms, a few tips to remember are:

1) Only ask questions that you absolutely need to ask. If you don't know
how you're going to use the information don't ask. The more questions the
lower the response.

2) Make your privacy stance clear. In Georgia Tech's Sixth Survey of
Internet Users, 70.2 percent of those who didn't register at sites said it
was because they were unclear about how the information would be used.

3) Provide users with an incentive to register. Otherwise, why should they
bother?
