A thing I wanted to tell you the last time this discussion popped up,
but somehow I forgot:

In Germany many sites measure their page views with a program called
ZMOD. This is an extension to the WWW-server (Apache, Netscape, IIS),
that sends a non-cachable 1x1 pixel gif image. This image is embedded
into every page, that should be counted. It's as uncacheable as
possible with "pragma no-cache" and "expires some time ago"-headers.
You can find a description on this on http://www.pz-online.de, but
it's in German only. The software is free by the way.

Our figures (we do a stock market simulation plus two finance related
magazines) show that in our game, where most pages are created
dynamically) about 10 % of the page views get stuck in some kind of
cache, in our magazine part (mostly static pages) we see a 35 %
increase.

A good friend of mine operates a proxy cache and he doubts that
76 % is possible. Normal proxies see hit rates on HTML-pages in the
area of 30 - 40 %. I think AOL figures match better ...
