Jane,
I have used Survey Pro from Apian for a long time. You can
contact them at apian.com. They have an add-on to the basic program
that will support the collection of data from online surveys.

The program is a combination of a survey design tool and reporting
systems. It is one of the original programs for survey analysis and
has lots of features. Very sophisticated and professional. Bill Ray,
the founder is a great guy and occasionally answers the help line
himself. I would give them a serious look.
