I also commented on this document and contacted the author twice to no
avail. I tested the proposed anti-caching headers (Expires plus Pragma plus
Cache-Control) and found out that it seems to be overkill (at least on
Netscape 3 and 4). Clicking the browser's back-button or even resizing the
browser window resulted in a banner-reload.

I first noticed this when we displayed AdBot banners at our site. Their
impression count was always *way* above our page counts. Sort of an early
make-good for the money they still owe us now :)
By telneting their ad server we found out that AdBot also used a
combination of Expires: and Pragma: headers.

We implemented a different anti-caching technique. As a Web publisher we of
course support the idea of cache busting. But counting a fresh banner when
a user resizes the window is going too far and has a very negative effect
on click through. Browser bugs (constant reloading of animated gifs) might
add to this problem.
