Some very good thoughts. I'd like to add a few comments (see below)...

That's great for simple one-page "flat" surveys. However, if skip
patterns, branching, randomization and data validation are factors of
importance to you, it is best to stick with web-based, with an email
recruit where possible.

Excellent!!!

Another consideration along those same lines is the carefully think about
which questions you force validation on. We have several customers (we
program surveys for a living) who make us validate EVERY question. That
is, the users are NEVER allowed to skip questions. As a result, those
clients experience very high mid-interview "drop out" rates. Only force a
response if it is absolutely necessary to the results of the survey.
Considering the low cost of online interviewing, it's much easier to make
up the difference with a larger sample size.
