The "major" networks, such as 24/7, Doubleclick, and
Flycast all sell almost any of those sizes for you,
however, since it's not typical that users do sell
inventory of that size, they don't sell a lot of
inventory.
Some other networks that sell this size include:

They offer .21 - .35 per click.

TeknoSurf sells on both a CPM and a CPC rate card.
