Usually through the use of custom programming.  

By creating a cgi-script that is called instead of the page
itself, a "dynamic" page can be returned in response to a
user's query.  This page can contain any number of changing
elements.

Most sites today use home-spun technology to handle the 
placement of rotating banner ads.  

We've developed a number of custom solutions to this problem
for our customers (see http://www.mediacentral.com) 

We also have a (soon to be released) advertising server
component (see http://newsspace.internet.com) that allows
placement of dynamic elements based upon a number of criteria
including search query string, geographic location of user and
time of day.

  Nope, they are buying impressions. To share some info I got, I talked 
to
two large sites we do barter deals with. One of them counts the number 
of
times the GIF is sent, as I am doing now. However, they have so many ads 
on
rotation, most people never see the same one twice in any single visit, 
so
the caching stuff isn't a problem. The other site makes 50 copies of the 
ad
and rotates those 50, so people who hit it again load a different copy. 

  Since we use fixed, non-rotating ads, that gives me three options. 
Report
the impressions as the same as the times the page was sent, use a CGI to
send the image instead (which should never be cached), or rotate a few 
dozen
copies of the same ad to mostly defeat caching/etc. The first is best 
for
the viewers, but advertisers don't trust that number. The second is best 
for
an accurate count (I suspect), but may be too much load on the server. 
The
third is just a hassle.

  I'll report back to the list whatever I decide to do, and how 
successful
it was or wasn't. I'm learning towards CGI.
